Cal-Ore Life Flight has over 16,000 participants who belong to the membership program. Combined ground
and air memberships are available to all interested parties. In addition, member
or not, residents throughout northern California and southern Oregon can preselect
Cal-Ore as their air ambulance provider.
Membership Benefits
The main benefit of membership
is a substantial cost savings on the price of any ground or air ambulance
transport. Cal-Ore accepts insurance reimbursement as payment in full, once members deductibles are
met. Members also receive other discounts on non-emergency or convenience ground and air transports and for non-transport services.

Acceptance of insurance (after satisfying deductibles) as payment in full for all ambulance transports (minimum reimbursement must meet 80% of the Medicare allowable amounts)

Membership Rules

I hereby apply for
membership for myself and my dependent family members listed on this
application. Cal-Ore Life Flight covers husband and wife and unmarried,
dependent children under the age of 23. Your child is a dependent if you
claim him or her as a dependent on your income tax return. If you do not, a
separate membership is necessary, even if the child lives at your residence.

I agree to transfer
directly to Cal-Ore Life Flight my rights to all insurance payments or other
applicable coverage due me for Cal-Ore Life Flight services.

Provided I have insurance
coverage, Cal-Ore Life Flight will accept payment from insurance carriers as
payment in full. I understand that I am responsible for insurance
deductibles.

In the event the member's
insurance provider denies the transport claim, or if the member has no
coverage, the member/patient will receive a substantial billing reduction.

I understand that this
membership provides for emergency medical care and transportation. I also
understand that Cal-Ore Life Flight membership is not insurance, but is in
addition to and secondary to any insurance or medical benefits I may have.

If non-transport services
are rendered, it is our experience that most insurance providers will not
cover these fees. Member patients receiving non-transport services not
covered by insurance will receive a 50% discount.

No refunds will be issued
on membership purchases.

Participation in the
Cal-Ore Life Flight Membership Plan may allow me, or any of my listed family
members, to a discount on "ELECTIVE" (non-physician authorized or
non-emergency) transportation provided only by Cal-Ore Life Flight. Elective
transports require scheduling and payment arrangements 24 hours in
advance since most insurance plans will not cover elective transports.

Cal-Ore Life Flight
transports patients based on medical need, not membership status, and
transports patients to the closest, medically appropriate facility.

All flights must be
ordered by a licensed physician.

There may be times, due to
weather or other circumstances beyond our control, that Cal-Ore Life Flight
cannot guarantee availability of the flight service.

"A Word from the California Department of Managed Health Care"
(A) "BEFORE YOU PURCHASE: If you are currently enrolled in a Health Maintenance Organization
(HMO) or other Health Insurance, the benefits provided by your HMO or other Health Insurance. If
you have a question regarding whether your HMO or other Health Insurance offers benefits for ambulance
services, you should contact that other company directly."
(B) "WARNING: This Ambulance Plan is not an insurance program. It will not compensate or reimburse
another ambulance company that provides emergency transportation to you or your family. This may occur
when the 911 Emergency System has indepently determined that another company could provide more
expiditious service or is next in the rotation to receive a call. This might also occur when the
Ambulance Plan is unable to perform within a medically appropriate time frame due to a mechanical
or maintenance problem or being on another call."
(C) "COMPLAINTS: For complaints regarding Cal-Ore Life Flight, first attempt to call us
at 1-800-761-5183. If Cal-Ore Life Flight fails to resolve the complaint to your satisfaction, contact
the Department of Managed Health Care at 1-800-400-0815. The Department's website is
http://www.dmhc.ca.gov. You may obtain complaint forms and instructions online."
(D) "OPERATING UNDER CONDITIONAL EXPEMPTION: Cal-Ore Life Flight is operating pursuant to an
exemption from the Knox-Keene Health Care Service Plan Act of 1975 (Health and Safety Code section 1340
et sep.)"
